Your stuff stays yours. The document is assembled by this page's JavaScript as you type. Names, prices, and photos never leave your machine, and the page keeps working with the network unplugged. Printing uses your browser's own print dialog - choose "Save as PDF" there to get a file.

The background check consent form is the one where the formatting is the law. In the United States the disclosure has to be a clear, standalone document - not a paragraph inside the application, not stapled to a liability release - and employers lose cases on exactly that point far more often than on the content of the check itself. So this form keeps the disclosure separate and plain, states in your own words what the report may cover rather than hiding behind the phrase consumer report, asks whether checks will be repeated during employment, and sets out the adverse-action promise the applicant is entitled to rely on. Fill in the employer, the role, and the applicant, and it prints as a document you can hand over on its own. Applicant names, dates of birth, and addresses are assembled in your browser and never uploaded, which is the correct handling for data you are about to send to a screening agency and nowhere else.

Questions this template gets asked

Why must the disclosure be a standalone document?

Because the Fair Credit Reporting Act requires it, and the courts have read that strictly. A disclosure combined with an application form, an arbitration clause, or - the classic mistake - a liability waiver, has repeatedly been held to violate the standalone requirement, and that is a technical failure that does not depend on the applicant being harmed.

Can the authorization be on the same page as the disclosure?

The disclosure must stand alone; the written authorization is commonly presented with it and is generally accepted in practice, which is how this form is laid out. What must not appear alongside the disclosure is extraneous material such as a release of liability. If your risk tolerance is low, present them as two pages.

What is adverse action and why is it on the form?

If something in a report may cost the applicant the job, they are entitled to a pre-adverse-action notice with a copy of the report and a summary of their rights, a reasonable window to dispute it, and then a final notice. Saying so on the authorization sets the expectation correctly and is a reasonable thing for an applicant to be told up front.

Do we need consent for checks repeated during employment?

If you intend to re-run checks later, say so at the outset - which is why the form has an ongoing-checks field. Relying on a years-old authorization for a fresh check is a weak position, and some jurisdictions require fresh consent regardless.

Is this background check consent form free to use?

Yes - free, no account, no watermark. It prints from your browser, and the applicant's name, date of birth, and address never touch a server. Because the rules here are technical and vary by state and city, have counsel review the wording before you adopt it.
