Field Trip Permission Slip
A school field trip permission slip: trip details, transport, guardian consent, medical and emergency contacts, and signatures. Prints from your browser; nothing uploaded.
Your stuff stays yours. The document is assembled by this page's JavaScript as you type. Names, prices, and photos never leave your machine, and the page keeps working with the network unplugged. Printing uses your browser's own print dialog - choose "Save as PDF" there to get a file.
The permission slip that comes back unsigned is usually the one that asked for a signature without answering the questions a parent actually has: where exactly, how are they getting there, what time do they get back, what does it cost, and who do I call. This form makes each of those a field, so the slip that goes home states the plan and the slip that comes back is consent to that plan rather than to an outing in general. It carries the two things staff genuinely need on the day - the medical and allergy notes, and authorization to seek treatment if a guardian cannot be reached - plus a due-back date printed on the signature line, because chasing slips is the actual work. It is also honest about what a permission slip is not: in most places it does not waive a school's duty of care, and this one does not pretend to. Student names, medical details, and family phone numbers never leave your browser.
Questions this template gets asked
What should a field trip permission slip include?
The destination and purpose, the date with departure and return times, how the students travel, any cost, the student and class, a parent or guardian with a phone number reachable during the trip, an alternative emergency contact, allergies and medication, authorization to seek emergency treatment, and a signature with a due-back date.
Does a permission slip release the school from liability?
In most jurisdictions, no - not for negligent supervision, which is the claim that actually arises. It records informed consent and gives staff the medical facts and the authority to act in an emergency. Slips written as blanket liability releases tend to be unenforceable against a school anyway, so this one does not try.
Why ask for a second emergency number?
Because the single listed guardian is the one person guaranteed to be unreachable at the moment you need them. An alternative contact who is not travelling on the trip is the difference between a five-minute problem and an hour-long one.
How long should signed slips be kept?
Carry them on the trip itself, with the roster, so the medical notes are physically present. Afterwards keep them in the student file for as long as your district or authority policy requires, which is commonly the school year at minimum and often longer.
Is this permission slip template free?
Yes - free, no account, no watermark. Fill it in, print one per student, or save as PDF from the print dialog to email to families. Student medical details and family phone numbers never touch a server.
