Your stuff stays yours. The document is assembled by this page's JavaScript as you type. Names, prices, and photos never leave your machine, and the page keeps working with the network unplugged. Printing uses your browser's own print dialog - choose "Save as PDF" there to get a file.

A piercing consent form earns its place at the two moments that actually cause trouble: before, when the marking has to be checked against what the client thinks they asked for, and afterwards, when healing goes badly and nobody can agree on what aftercare was given. Fill in the studio, piercer, client, placement, and jewellery, and the release on the right assembles with an ID and age block, a medical disclosure section written around the conditions that genuinely change healing, eight initial-each acknowledgments, and signature lines for client and piercer. The jewellery field is on the document deliberately - material and gauge are the first questions asked when a site reacts, and a form that recorded implant-grade titanium at 16g answers them. Client names, birth dates, and disclosed conditions are assembled by this page's JavaScript in your browser and never uploaded.

Questions this template gets asked

What is the minimum age for a piercing?

It is set locally and it is not the same rule as tattooing. Many places allow certain piercings for minors with a parent present and signing, while restricting others - navel, nipple, genital, and sometimes cartilage - to adults regardless of parental consent. This form is written for clients 18 and over. Check your health authority rule before offering a minor version.

What should a client disclose before being pierced?

Anything affecting bleeding, healing, or infection risk: diabetes, blood thinners or haemophilia, heart valve conditions, a tendency to keloid, skin conditions at the site, allergies including nickel and latex, pregnancy or nursing, and current medication. Nickel allergy in particular is the one that turns a routine piercing into months of irritation.

Does the form need to record the jewellery?

It is one of the most useful lines on it. Material and gauge determine reaction risk and healing, and if a client returns with an angry site, knowing exactly what was placed - and that it was implant-grade rather than costume metal - resolves most of the question immediately.

How long should studios keep signed forms?

At least your jurisdiction's limitation period for personal-injury claims, and at least any minimum retention your health department sets, whichever is longer. Inspectors can ask to see consent records, so keep them ordered and retrievable rather than in a drawer.

Is this piercing consent form free to use?

Yes. No account, no watermark, no paywall. The form builds in your browser as you type and prints from the browser's own dialog. The client's personal and medical details never touch a server.
